Prospective Graduate Student Forms

We want you to access the resources you need with ease. This page will provide direct access to relevant and important forms that you need for your successful journey pursuing a graduate degree at 黑料网.

Update Term Form

This Form is used for when:

    • You were accepted as a graduate student but did not enroll and would like to be considered for a future semester.
    • You submitted a graduate application but now need to change your application term.
    • You are allowed only one update and the update must be within one academic year from the original application term.
      • If you complete the Update Form, please do not create a new application.
      • If you already used the Update Form or it is past one academic year, please complete a new application. 

Deadlines:

    • Fall Semester: July 1
    • Spring Semester: November 1
    • Summer Semester: April 1

This form is NOT to be used if you wish to change your program of study.

Request for Waiver of TOEFL/IELTS Test

Complete this form if you would like to request that the TOEFL/IELTS test be waived.

Program Change Request

If you are an admitted student to the Graduate College in any program, and have not enrolled in that program, you may request a Program Change.

If you request a Program Change, your application will be updated to reflect the new program鈥檚 requirements, and you will need to go through the admission process again.

Graduate Application Readmission

If you were enrolled in graduate classes, but have not enrolled for at least three consecutive semesters, including Summer term, and wish to be readmitted, submit this form.
